  • Community Feedback: Health Assessment Survey
We need your help identifying the health needs of our community! Effingham County is conducting a project aimed at assessing the most important issues and priorities that affect the health of the community. The purpose of this survey is to get your opinions about community health issues and quality of life as county residents. In collaboration with our community partners, we plan to compile this information and use it as input for the development of a community health improvement plan which will allow us to target ways in which we can improve the health of all residents. All responses will be anonymous.

Please participate and share your health concerns for Effingham County by responding to this short (5 minute) survey. Thank you for your time and interest in helping us to identify our most pressing problems and issues!
